Treating HFpEF: Huff-Puff without a Solution?

  1. Other treatable causes for heart failure like HT, CAD, AF have to be identified and treated.
  2. Medications like ACEIs, ARBs, β blockers, MRA, which are  useful in the treatment of HFrEF do not improve outcomes in HFpEF.
  3. However careful assessment of volume overload, treatment with diuretics,  small dose β-blocker and ARB are useful in reducing hospitalization in a minority of patients.
  4. PARAGON-HF trial may give insight in the use of ARNI in the management of the same.
